Graduate Photo & Video Marketing Consent

I hereby give permission to:

This is a separate, voluntary consent being signed now that I am a graduate/alumnus of FVI. Whether or not I signed any photo/video release during enrollment, by signing this form I am confirming that FVI may use my image, likeness, and/or interview statements in marketing and promotional materials that feature graduates/alumni. This consent is not required for graduation or receipt of any services. This form reflects my current preference and supersedes any prior photo/video consent selection for graduate/alumni marketing use.
Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to FVI School of Nursing and Technology, its affiliates, contractors, and agents, to record and use my image and likeness and/or any interview statements from me in its publications, advertising, or other media activities, including the internet and social media.

This consent includes, but it is not limited to:

a) Permission to interview, film, photograph, tape, livestream, or otherwise make a video/audio reproduction of me and/or record my voice;

b) Permission to use my name (or first name only) and/or identify me as a graduate/alumnus of FVI;

c) Permission to use quotes (or excerpts), photographs, video, and/or recordings of my voice, in part or in whole, in print and digital media (including websites, email, and social platforms), television, radio, and other electronic or theatrical media, and in mailings for educational, awareness, and promotional purposes;

d) Permission for FVI to edit, crop, caption, or combine these materials with other content (for example, music, graphics, or text) without prior approval;

e) I will make no monetary or other claim, including any and all claims for libel, invasion of privacy, or right of publicity, for the use of the interview and/or the photograph(s)/video(s)/recording(s) of my voice.
